Healthcare Transactions Group, a healthcare mergers and acquisitions advisory firm in the post-acute sector, arranged the leasing of Sagepoint Nursing and Rehabilitation Center, a 170-bed licensed Skilled Nursing Facility in La Plata, Maryland (Charles County). The transaction closed on April 1, 2024. The facility will be leased from Sagepoint Senior Services, a not-for-profit organization which will continue to own the campus.
The SNF is a one-story, 67,228 square-foot building (with a basement) on a 16.8-acre campus; built in 1976 with a 105-patient capacity, the structure was expanded in 1992 to serve 170 patients. In 2017, more private rooms and enhanced rehabilitation services were added. Forty-five patient rooms are private, and sixty-three are semi-private.
The La Plata campus includes the SNF, an adult day care center, a medical office building with a Fresenius-operated dialysis center, and an assisted living facility. Sagepoint Senior Services will continue to own and operate the medical office building, adult day care center, and assisted living facility.
The lease includes a purchase option subject to certain terms and conditions. The lessee was selected after a rigorous two-round auction and selection process of potential partners for the campus as determined by Sagepoint’s Board of Directors.
Healthcare Transactions Group initiated the transaction while serving as the exclusive mergers and acquisitions advisor to Sagepoint Senior Services. The Sagepoint deal represents Healthcare Transactions Group’s twenty-eighth post-acute transaction in Maryland.
