Owings Mills, MD, July 7, 2025 – Healthcare Transactions Group is pleased to announce that it arranged the sale of Sagepoint Nursing and Rehabilitation Center, a 170-bed licensed skilled nursing facility in La Plata, Maryland (Charles County). The SNF is a one-story, 67,228 square-foot building (with a basement) on a 16.8-acre campus.  The acquirer, Green Tree Management Company, has been leasing the facility since April 2024 from Sagepoint Senior Services, a not-for-profit organization that will continue to own and operate the remaining campus facilities.  Healthcare Transactions Group also previously arranged the lease. The transaction closed on June 23, 2025. Price and other details of the transaction are confidential.

The completion of the sale of Sagepoint marked more than two years of Healthcare Transactions Group advising Sagepoint Senior Services through significant challenges, including marketing the facility for sale amid ongoing litigation, negotiating an interim lease, a transition in Maryland’s Medicaid reimbursement methodology, Sagepoint’s bankruptcy filing, and addressing a potential future subdivision of the campus to enable the new owner to qualify for HUD financing.

The La Plata campus includes the SNF, an adult day care center, a medical office building with a Fresenius-operated dialysis center, and an assisted living facility.  Sagepoint Senior Services will continue to own and operate the medical office building, adult day care center, and assisted living facility.
